We’d like to start off by saying welcome and thank you for considering Sunny Smile Dental as your go-to dental office for comprehensive dental services. As you get acquainted with our website, we hope that you take a moment to learn how to prepare for your future dental visit. Below, you’ll find info on financing care, new patient forms, what to expect during your first visit; everything you need to become a brand-new patient at Sunny Smile Dental!
When you first walk into the dental office, you’ll be greeted by our friendly front desk team member, who will then ask you to fill out relevant new patient information. If you already filled out your new patient forms prior to arrival, you’ll be asked to wait for a few minutes until Dr. Lee is ready for you. Once you are escorted to the operatory, you’ll have X-rays taken as needed, meet with Dr. Lee to discuss your needs, and get your treatment plan created if any problems are present.
In order to best determine your specific needs, we need to collect information about you regarding your current oral and overall health. If you have any conditions that could affect your oral health or you take certain medications, your new patient forms give you a chance to let us know ahead of time. If you’d prefer to expedite your first visit, please click the link below, and fill out your patient form below before you arrive.
Dental insurance can make paying for your most basic treatments more affordable and your costs more predictable. Dr. Lee is in-network with most dental insurance PPO plans and is more than happy to help you understand your coverage before you begin any treatment. Our financial coordinator will also help you schedule treatments based on what works for you. If you have questions regarding your insurance, we’d love to answer them.

Do you need help paying for your next dental treatment? At Sunny Smile Dental, we also encourage patients to take advantage of third-party financing companies like CareCredit if their insurance doesn’t cover all of their treatment costs or if they need help paying for treatments not typically covered by insurance. In most cases, you can pay for care at little to zero interest, making the process even more affordable. See if you qualify by following the link below!
